Does it ask questions first?

Yes, a few at a time. It asks what the tool should handle, for examples of how you would use it, and what requests should trigger it, then stops asking once the purpose is clear.

What if the name is already taken?

Tool names are unique in your account. If you already have a custom tool with the same name, askROI asks you for a different one before saving.

How is this different from the AI Mini App Builder?

The mini app builder makes an interactive app you click through inside the chat. The custom tool builder makes a set of instructions that shapes how askROI handles your future requests; it has no screen of its own.

It only creates a tool when you ask for one, and it does not connect new apps or accounts on your behalf. A custom tool changes how askROI works on a task; it does not run by itself on a schedule.
