- How is the risk scored?
- Severity runs from negligible (1) to critical (5), and likelihood from remote (1) to almost certain (5). The score is severity times likelihood: 1 to 4 is green, 5 to 9 yellow, 10 to 15 orange, and 16 to 25 red, and each level comes with recommended actions.
- What is in the memo?
- The risk description, background, severity and likelihood with rationale, the score and level, contributing and mitigating factors, a table of mitigation options with effectiveness and effort, a recommended approach, the residual risk, a monitoring plan, and next steps with owners.
- When does it recommend outside counsel?
